PHYSICAL THERAPY POLICIES

PATIENT BILLING/PAYMENT POLICIES

  • Payment or a valid Credit Card on file must are required to schedule Physical Therapy Sessions.

    • Pre-payments and credit card charges will be applied following service or in case of a no show or late cancellation.

    • Patient’s can manage their billing/payment information, session cancellations and view session/class history and purchases by login into their Account.

PATIENT CANCELLATION POLICIES

  • Appointments must be cancelled 24-hours prior to scheduled appointment time to avoid being charged.

    • Late cancellations and no-shows will be charged the full amount of the booked appointment.

    • If a patient arrives more than 15 minutes late for an appointment without notification of tardiness, treatment may be considered a late cancellation. The patient understands and agrees that they may not be seen, as their appointment time may then be filled, but they will be charged as a late cancellation for the full amount of the booked appointment.

    • Patients that arrive late for a scheduled appointment start time will still be charged the full amount of the booked appointment regardless of a shorter session duration. Time missed during a session DOES NOT roll over to other session times.

    • Patient's will be given ONE (1) emergency or severe/contagious illness cancellation leniency per calendar year if patient provides notification prior to scheduled appointment start time or can provide documentation following scheduled appointment.

      • Cancellation leniencies will be credited to the patient’s account for the price of scheduled appointment. if rescheduling requires full priced services but the leniency cancellation was originally scheduled at a discounted price, then the remaining balance will be due at time of service.

    • All cancellations and absences will be documented in patient medical records.

PATIENT ATTENDANCE POLICY

  • TWO (2) consecutive absences without 24-hour advanced notice may result in the cancellation of all remaining scheduled physical therapy appointments; as such failures may negatively impact your treatment plan.

    • Repeated failure to comply with this policy will result in your appointments being scheduled based on availability, which will require you to CALL for available appointments on the day you seek to receive therapy.

PHYSICAL THERAPIST CANCELLATION POLICY

  • We will attempt to reschedule you with another therapist if possible, or reschedule your appointment at your earliest convenience. In case of a therapist canceling an appointment(s), no charge or fee will occur and all affected patient(s) will be notified immediately.
